Dear Palmer Families,
We are pleased to inform you that we are partnering with Vision Coverage of Colorado to provide eyecare services and treatment plans at Palmer Elementary School on Tuesday, October 8, for one day only.
The Vision Coverage of Colorado (VCC) will accept Health First Colorado (Medicaid), and CHP+, The Vision Coverage of Colorado is also able to see students without insurance at NO COST (VCC will never send a bill for glasses or services). If you have commercial insurance such as VSP, Blue Cross Blue Shield, etc, this is not for you.
Attached is the consent form to allow Vision Coverage of Colorado to provide eyecare services and treatment plans, and share medical information about your child. You DO NOT need to be on campus as long as you signed the consent form. One form is required for each student receiving services.
